To request a copy of a Sacramental Certificate for a Sacrament that was received at IHM, please use the  appropriate request form below.  

• Certificates will be mailed to the requestor. Please allow 2 weeks for delivery. 
• A Certificate may be picked up at the Rectory if it is needed quickly.  Please note in the comments field on the request form if a "rush" is needed.
• The suggested offering per certificate is $20.00.  Please mail or drop off your payment to the Rectory.
